What is Medicare eligibility?
Skilled Need: A skilled need is a need for either skilled nursing or physical therapy as communicated by the Conditions of Participation for Medicare and Medicaid Services.
Physician's Order: A physician must direct orders to start care and re-certify the orders for home health services.
Homebound: The person must be considered homebound. Homebound is defined as a taxing - effort to leave the home, assistance of supportive devices such as a cane, walker or wheelchair and the support of another person.
What happens when a referral is made?
When a referral comes into the office, a nurse or a physical therapist completes an assessment. From that assessment a care plan is created. The care plan defines what types of services are needed and at what frequency. For example, services could be a home health aide three times a week and a nurse twice a week. Coordination of care will be managed with your physician and your Overlook Visiting Nurse Association team.

Who pays for home care services?
Home care is paid for by Medicare, Medicaid, Private Insurance, Veterans Benefits, Long Term Care Insurance, and Private Pay, as well as many managed care insurances.
Can I hire caregivers if my residence is an Assisted Living Facility?
Yes, where you call home is where services can be provided.
